Job Description

MDS Coordinator Job Description:

We are seeking a detail-oriented, knowledgeable, and compassionate MDS Coordinator to join our interdisciplinary team. The MDS Coordinator will be responsible for overseeing and coordinating the completion of the Minimum Data Set (MDS) assessments in compliance with federal, state, and facility guidelines. This role is critical in ensuring accurate clinical documentation and driving care planning, reimbursement, and quality improvement efforts.

MDS Coordinator Job Responsibilities:

  • Coordinate and complete MDS assessments, ensuring accuracy and timeliness.
  • Monitor and track MDS schedules to maintain compliance with CMS and NYS Department of Health regulations.
  • Collaborate with nursing staff, rehabilitation, social work, dietary, and other departments to ensure comprehensive and accurate resident assessments.
  • Provide education and support to staff regarding MDS processes and requirements.
  • Participate in care plan meetings and assist in developing individualized resident care plans.
  • Review, analyze, and validate MDS data for accuracy, making corrections as needed.
  • Support quality improvement initiatives by identifying trends and assisting with data-driven solutions.

MDS Coordinator Requirements:

  • Current RN license in the State of New York (RN preferred).
  • Minimum of 2 years of experience in a skilled nursing facility, with at least 1 year of MDS coordination experience preferred.
  • Strong knowledge of CMS regulations, RAI process, and PDPM.
  • Excellent organizational, communication, and problem-solving skills.
  • Ability to work collaboratively in a team environment.
  • Proficiency in electronic medical record systems (experience with SigmaCare/MatrixCare a plus).
